Thyroid disorders are significantly more common in women than men, and symptoms can be easy to dismiss as normal fatigue, stress, or aging. Because an underactive thyroid (hypothyroidism) and an overactive thyroid (hyperthyroidism) can produce very different — sometimes opposite — symptoms, recognizing the pattern matters.
Signs of an Underactive Thyroid (Hypothyroidism)
- Persistent fatigue that doesn’t improve with rest
- Unexplained weight gain despite no major change in diet or activity
- Feeling cold more often than those around you
- Dry skin, brittle nails, and hair thinning
- Heavier or irregular menstrual periods
- Constipation and slowed digestion
Signs of an Overactive Thyroid (Hyperthyroidism)
- Unexplained weight loss despite normal or increased appetite
- Rapid heartbeat or palpitations
- Feeling unusually warm or sweating more than usual
- Anxiety, irritability, or trouble sleeping
- Lighter or less frequent menstrual periods
- Hand tremors
Why Women Are More Affected
Thyroid conditions, especially autoimmune types like Hashimoto’s thyroiditis and Graves’ disease, occur significantly more often in women than men, likely related to immune system differences and hormonal factors. Pregnancy and postpartum periods are also times when thyroid function can shift, making these life stages particularly important for monitoring symptoms.
How Thyroid Problems Are Diagnosed
A simple blood test measuring thyroid-stimulating hormone (TSH), and sometimes additional hormones like free T4, is the standard first step. Because symptoms overlap with many other conditions, testing is the only reliable way to confirm a thyroid issue rather than guessing based on symptoms alone.
When to See a Doctor
If you’re experiencing a cluster of these symptoms — particularly unexplained weight changes, persistent fatigue, or menstrual cycle changes — it’s worth requesting thyroid testing, especially if you have a family history of thyroid conditions.
Thyroid Changes During Pregnancy and Postpartum
Pregnancy naturally shifts thyroid hormone demands, and some women develop thyroid dysfunction for the first time during pregnancy or in the months after delivery (postpartum thyroiditis). Because symptoms like fatigue and mood changes overlap heavily with normal pregnancy and new-parent exhaustion, thyroid problems during this period can be especially easy to miss without specific testing, which is one reason thyroid function is often checked as part of routine prenatal and postpartum care.
Treatment Overview
Hypothyroidism is typically treated with a daily synthetic thyroid hormone replacement, taken consistently and monitored with periodic blood tests to fine-tune the dose. Hyperthyroidism treatment varies more by cause and severity, ranging from medications that reduce thyroid hormone production to, in some cases, radioactive iodine treatment or surgery. Both conditions are generally very manageable once properly diagnosed and treated, which underscores the value of getting tested rather than living with unexplained symptoms.
Living Well With a Thyroid Condition
Once diagnosed and on appropriate treatment, most women with thyroid conditions can expect to feel significantly better within weeks to a few months, though finding the right medication dose sometimes takes a bit of adjustment. Routine follow-up blood tests, typically every few months initially and then annually once stable, help ensure the dose stays appropriate over time, since thyroid hormone needs can shift with age, weight changes, pregnancy, and other factors.
Thyroid Nodules and Other Related Concerns
Separately from overall thyroid hormone levels, some women develop thyroid nodules — small lumps within the thyroid gland — which are usually noncancerous but are typically evaluated with an ultrasound and sometimes a biopsy if a doctor notices one during an exam or imaging. Finding a nodule isn’t the same as having a thyroid hormone imbalance, and the two are generally assessed and managed separately.

Frequently Asked Questions
Why are thyroid conditions more common in women?
Autoimmune thyroid conditions, which make up a large share of thyroid disorders, occur more frequently in women, likely due to a combination of immune system and hormonal factors that aren’t fully understood.
Can thyroid problems affect periods?
Yes, both an underactive and overactive thyroid can change menstrual cycle patterns, from heavier and more frequent periods to lighter or missed ones.
What’s the first test for thyroid problems?
A TSH (thyroid-stimulating hormone) blood test is typically the first step doctors use to screen for thyroid dysfunction.
Can thyroid problems affect fertility?
Yes, both an underactive and overactive thyroid can affect ovulation and fertility, which is one more reason to get tested if you are trying to conceive and experiencing symptoms.
