High cortisol in women often shows up as a combination of weight gain around the midsection and face, irregular periods, mood swings, and trouble sleeping — but confirming it requires blood, saliva, or urine testing, since these symptoms overlap with many other conditions.
What Cortisol Does
Cortisol is the body’s primary stress hormone, released by the adrenal glands. It helps regulate blood sugar, metabolism, and the body’s stress response. Chronically elevated cortisol — whether from prolonged stress, certain medications, or an underlying condition like Cushing’s syndrome — can affect nearly every system in the body.
Physical Symptoms
- Weight gain — particularly around the abdomen, face (sometimes called a “moon face”), and upper back
- Thinning skin and easy bruising
- Stretch marks — often purple or pink, wider than typical stretch marks
- Muscle weakness, especially in the arms and legs
- High blood pressure
- Irregular or missed periods
Mood and Sleep-Related Symptoms
- Increased anxiety or irritability
- Difficulty falling or staying asleep
- Low mood or feeling emotionally flat
- Brain fog or trouble concentrating
Common Causes of Elevated Cortisol
- Chronic, unmanaged stress
- Long-term use of corticosteroid medications
- Pituitary or adrenal gland tumors (less common, but a key reason to test rather than self-diagnose)
- Poor sleep quality over extended periods
How It’s Diagnosed
Doctors typically use a combination of blood tests, 24-hour urine cortisol tests, or saliva tests taken at specific times of day, since cortisol naturally fluctuates. Because levels vary so much based on time, stress, and recent activity, self-diagnosing from symptoms alone isn’t reliable.
What Can Help Lower Cortisol Naturally
- Consistent sleep schedule and prioritizing 7-9 hours per night
- Regular moderate exercise (overtraining can actually raise cortisol further)
- Stress-reduction practices like breathing exercises or mindfulness
- Limiting caffeine, particularly later in the day
If a medical cause like Cushing’s syndrome is suspected, lifestyle changes alone won’t resolve it — proper diagnosis and treatment from an endocrinologist is necessary.

Frequently Asked Questions
Can stress alone cause permanently high cortisol?
Chronic stress can keep cortisol elevated for extended periods, but persistently high levels should still be evaluated to rule out an underlying medical cause.
Does high cortisol always cause weight gain?
It’s a common symptom but not universal — some people with elevated cortisol don’t notice significant weight changes.
What test confirms high cortisol?
A 24-hour urine cortisol test or late-night salivary cortisol test are commonly used, often alongside blood tests, depending on what your doctor suspects.
This is general information, not a diagnosis. See a doctor if you’re experiencing these symptoms persistently.
