Becoming an Army combat medic is one of the more direct paths into military medicine, requiring no prior healthcare background to start. There are two main routes worth understanding: the standard 68W Combat Medic Specialist path, and the more advanced 18D Special Forces Medical Sergeant path for soldiers who go on to join Special Forces.
The Standard Path: 68W Combat Medic Specialist
- Enlist and complete Basic Combat Training, the standard entry point for all Army enlisted roles
- Attend the Combat Medic Specialist (AIT) course at Fort Sam Houston, which combines civilian EMT-Basic certification with military trauma care training
- Meet medical and fitness standards required for the role, including vision and physical fitness benchmarks
The Advanced Path: 18D Special Forces Medical Sergeant
18D is a far more advanced and selective role, open only to soldiers who have already passed Special Forces Assessment and Selection (SFAS) and gone through the Special Forces Qualification Course. The medical training for 18D is significantly longer and deeper than 68W training, covering advanced trauma care, minor surgical procedures, and even some dental and veterinary care, since Special Forces medics often operate without nearby medical facilities.
What the Training Actually Covers
- Tactical Combat Casualty Care, the foundation of all combat medical training
- EMT-Basic (or higher) certification, giving a civilian-recognized medical credential
- Extended field medicine for 18D candidates, including advanced procedures not typically covered at the 68W level
Requirements and Qualifications
- U.S. citizenship (or legal permanent residency, depending on current enlistment policy)
- Meeting ASVAB score requirements for the medical specialist track
- Passing a medical and physical fitness evaluation
- For 18D specifically, successfully completing SFAS and the Special Forces pipeline first
What Comes After
Combat medics can serve in a variety of assignments, from conventional infantry and support units to, for 18D medics, Special Forces operational detachments. Many use the EMT certification and trauma experience as a foundation for civilian paramedic work, nursing, or physician assistant programs after their service.
Pay and Career Outlook
Combat medics are paid according to the standard enlisted pay scale based on rank and time in service, with additional specialty or hazard pay possible depending on assignment. Beyond immediate pay, the certifications earned — particularly EMT credentials and, for 18D soldiers, advanced medical training — carry real value in the civilian job market, giving many veterans a head start into paramedic, nursing, or physician assistant careers after their service commitment ends.
Washout and Initial Challenges
New combat medics often describe the Combat Medic Specialist course as one of the more academically demanding parts of early Army training, combining the pace of EMT school with military discipline and physical training simultaneously. Attrition can occur for those who struggle with the coursework or physical demands, though the Army provides structured support, study time, and remediation opportunities for soldiers who need extra help passing certification exams.
Life Immediately After Training
Newly certified combat medics are typically assigned to a unit where they serve as the primary medical provider for a platoon or company, working under the supervision of a physician assistant or medical officer. Early assignments often combine routine clinic duties with continued hands-on training, giving new medics time to build confidence before taking on more independent responsibility during exercises or deployments.
