Testosterone naturally declines with age, but when levels drop below a healthy range, it can produce a noticeable set of physical, sexual, and emotional symptoms. Recognizing the signs early can help men get properly tested rather than attributing every symptom to “just getting older.”
Physical Signs
- Persistent fatigue that doesn’t improve with adequate sleep
- Loss of muscle mass and strength, even with consistent training
- Increased body fat, particularly around the midsection
- Reduced bone density over time, increasing fracture risk
Sexual Health Signs
- Reduced libido or interest in sexual activity
- Erectile dysfunction or difficulty maintaining erections
- Lower sperm count, which can affect fertility
Mood and Cognitive Signs
- Low mood or irritability
- Difficulty concentrating or a general sense of mental fog
- Reduced motivation or confidence
What Causes Low Testosterone
Testosterone naturally declines gradually after about age 30, but a drop significant enough to cause symptoms can also result from conditions like obesity, type 2 diabetes, certain medications, pituitary or testicular disorders, chronic stress, and poor sleep. Identifying the underlying cause matters for figuring out the right treatment approach.
How Low Testosterone Is Diagnosed
A simple blood test, usually done in the morning when testosterone levels are at their natural peak, can measure total and sometimes free testosterone. Because levels can fluctuate, doctors often confirm a low reading with a second test before making a diagnosis or discussing treatment options.
Lifestyle Factors That Influence Testosterone
- Poor sleep quality or insufficient sleep, which is strongly linked to lower testosterone levels
- Excess body fat, particularly visceral fat, which can affect hormone balance
- Chronic high stress, which elevates cortisol in a way that can suppress testosterone production
- Heavy alcohol use, which can directly impair testosterone production over time
- Sedentary lifestyle, since regular strength training is associated with healthier testosterone levels
Treatment Options
When low testosterone is confirmed and causing symptoms, treatment options range from addressing underlying contributing factors (weight, sleep, alcohol use) to testosterone replacement therapy in appropriate cases. Testosterone replacement carries its own risks and isn’t right for everyone, including men with certain types of prostate cancer or uncontrolled sleep apnea, which is why it requires a doctor’s evaluation and ongoing monitoring rather than self-treatment with over-the-counter “testosterone boosting” supplements, most of which have little evidence behind them.
How Common Is Low Testosterone?
Low testosterone becomes increasingly common with age, affecting a meaningful percentage of men over 45 and a higher share of men over 65, though it can occur at younger ages due to underlying medical conditions, obesity, or certain medications. Because symptoms overlap with normal aging, fatigue from poor sleep, and even depression, many men with clinically low testosterone go undiagnosed for years simply because they assume their symptoms are an inevitable part of getting older.
Why Self-Treating Isn’t a Good Idea
Over-the-counter “testosterone boosting” supplements are poorly regulated, often contain unproven ingredients, and in some cases have been found to contain undisclosed hormones or banned substances. Starting testosterone replacement without proper testing and monitoring also carries real risks, including effects on fertility, red blood cell counts, and prostate health, which is why this is a condition best managed with a doctor’s involvement from diagnosis through treatment.

Frequently Asked Questions
At what age does testosterone start to decline?
Testosterone typically begins a gradual decline after around age 30, continuing slowly through the following decades for most men.
Can lifestyle changes raise low testosterone?
In some cases, especially when obesity, poor sleep, or inactivity are contributing factors, improving these areas can help; however, clinically low testosterone often needs a doctor’s evaluation for an appropriate treatment plan.
What test confirms low testosterone?
A morning blood test measuring total testosterone, often repeated to confirm results, is the standard way doctors diagnose low testosterone.
Is low testosterone the same as andropause?
“Andropause” is a popular term sometimes used to describe age-related testosterone decline, but it is not an official medical diagnosis the way menopause is for women.
This is general information, not medical advice. See a doctor for testing and an accurate diagnosis if you’re experiencing these symptoms.
